As a Patient Advocate you will be responsible for supporting our Nurse Coaches by outreaching to patients to explain our Readmissions Program, in an effort to help prevent them from readmitting to hospitals or facilities post discharge. You will be reviewing and preparing data and demographics on patients recently discharged from the hospital prior to an initial assessment call with a Nurse Coach.


Schedule Options: 

Tuesday-Saturday 9am-6pm EST (Sun/Mon OFF) or Sun-Thursday 9am-6pm EST ( Fri/Sat OFF)

Start Date – Monday, March 18th, 2024

Hourly Rate - $18/hr

Training & Nesting Period – 3 to 5 Weeks
Training & Nesting Hours – Monday – Friday 8:30am – 5:30pm EST

Available Shift After Training & Nesting: Tuesday-Saturday 9am-6pm EST (Sun/Mon OFF) OR Sun-Thursday 9am-6pm EST ( Fri/Sat OFF)



In this role, you will:

• Collect and document all required data on patients recently discharged from the hospital to prepare for an outreach to the patient, connecting them to a Nurse Coach for an initial assessment.
• Responsible for servicing patients making incoming calls with questions and/or returning messages for their Nurse Coach.
• Provide outreach to patients to check in on non-clinical issue resolution and escalation of issues to Nurse Coaches or management when appropriate.
• Based on business needs, evening and weekend coverage may be needed.


You should get in touch if you have: 

  • High School Diploma or the equivalent.
  • Minimum of one year medical terminology, healthcare or medical services experience required.
  • Experience in a call center environment preferred.
  • Proficient data entry skills.
  • This position requires strong interpersonal and communication skills, customer and problem solving skills, as well as the ability to interact with all levels of management and a highly diverse environment.
  • Strong organizational skills and able to effectively manage and prioritize tasks.
