This position supervises a team of non-exempt associates within assigned unit in Operations. Monitors performance and productivity of individuals and the team and takes appropriate action to ensure department goals are met including, but not limited to regular scheduled and just-in-time coaching, recommending re-training, performance management and mentoring associates. Manages payer, provider and patient complaint resolution. Consistently identifies areas of opportunity/process improvement within department and recommends solutions as necessary.
In this role, you will work closely with the Provider Network Contract Management team and analyze / uncover critical performance trends that drive department and company performance. You will assess market and business trends to identify opportunities and risks in the provider network and work with large data sets to make recommendations that help drive business decisions.
As a Healthcare Customer Service Expert, you have the opportunity to be the first point of contact for patients needing home healthcare services. You will be responsible for receiving and responding to incoming calls from providers, referral sources and potential patients by collecting and entering clinical and demographic information into our CareCentrix portal. Typical phone calls range from interactions with customers who need basic information, to assisting those with more complex requests. The more challenging calls can be the most rewarding because you know you are making a difference to a customer or patient that needs your help.

The PAC Nurse is a telephonic position responsible for managing the length of stay (LOS) for Long Term Acute Hospital (LTACH), Skilled Nursing Facility (SNF), and Institutional Rehab Facility (IRF) for their assigned post-acute care facilities through collaboration PAC Nurse will also collaborate with key facility personnel as well as with CareCentrix internal Medical Directors, Market Engagement Directors and Nurse Managers to develop and maintain a timely discharge plan.
The Customer Advocacy Specialist incumbent serves as a critical subject matter expert (SME) role in driving resolution for internal or external issues or complaints. He or she will be able to de-escalate and resolve issues with a high level of independence. This role assists the Supervisor/Manager with coaching, on-the-job training, monitoring work, mentoring new associates and driving results. This position may also serve as a back-up in production. As a Sr. Analyst - Information Security, you will be responsible for the day-to-day administration of information security tools and devices. You will analyze new/existing security threats and implement security measures to safeguard against them. You will be tasked with establishing and updating actionable security metrics, monitoring information security service requests and associated ticket queues, and triaging response to suspected phishing attacks, security incidents, etc.
The individual in this position will at times interact closely with product vendors, service providers, and personnel from various IT and business departments — including application development, operations and network, privacy/compliance and business teams. This role requires a core working knowledge of operating system security, basic network protocols, and the security toolsets and applications utilized by the CareCentrix Information Security team.

OVERVIEW
As a Patient Advocate you will be responsible for supporting our Nurse Coaches by outreaching to patients to explain our Readmissions Program, in an effort to help prevent them from readmitting to hospitals or facilities post discharge.
