As a Senior Data Analyst you will develop, update, and maintain all operational tools used to standardize work. Clinical Reviewers are responsible for the Utilization Management and review of clinical information for service requests received and to render a medical necessity approval or recommended denial to the Medical Director. As part of this process, you will evaluate information concerning patient care and match those needs with available care options, consistent with CareCentrix guidelines and specific plan payer criteria.
OVERVIEW
As a Patient Advocate, you will work in a high-volume call center environment making outbound calls to patients recently discharged from the hospital and answering inbound calls from a queue.
You will ask non-clinical triage questions to determine if the Nurse Coach program could be beneficial to at home recovery or if other services are needed, such as transportation to doctors’ appointments. You will collect and document required data for end-to-end care to support the clinical program goals. For patients further along in the program, you will contact patients to ensure successful recovering at home.

As Lead Software Developer, you will have responsibility for leading day to day development of software integration components. You will work with CareCentrix Business Technology associates, as well as vendor partners and consultants in a fast paced, dynamic agile environment. You must be a hands-on technical resource comfortable ‘rolling up his/her sleeves’ to help the team design, develop, and test integration software. The UDH Platform initiative will include a completely new approach to developing software within CareCentrix, using the latest design approaches, technologies, and processes.
Post-Acute Care is an end-to-end post-acute offering that manages up to 90-day episodes of care, beginning prior to the patient’s discharge from the hospital, or for elective surgical cases, outreach begins prior to hospitalization. The program optimizes our home-based network, identifies the likely best site-of-care for the patient, manages length of stay (LOS) if a Skilled Nursing Facility (SNF) is appropriate, and reduces hospital readmissions. Our program also coordinates all of the services required for a patient to transition to their home faster and safer, via our network of home health, durable medical equipment and home infusion providers, all of whom are supported by our CareCentrix care coordination team.
This position provides strategic leadership and clinical operational oversight of the Post-Acute Care Readmission Management Program. In addition, this position provides product development and sales support for new readmission management programs and business expansion efforts within CareCentrix. This position functions as the Supervisor of the Post-Acute Care team and primary point of contact for day to day clinical questions, concerns from internal and external clients and provides team leadership and guidance regarding patient centered interventions. The PAC Nurse is a telephonic position responsible for managing the length of stay (LOS) for Long Term Acute Hospital (LTACH), Skilled Nursing Facility (SNF), and Institutional Rehab Facility (IRF) for their assigned post-acute care facilities through collaboration PAC Nurse will also collaborate with key facility personnel as well as with CareCentrix internal Medical Directors, Market Engagement Directors and Nurse Managers to develop and maintain a timely discharge plan.

As a Healthcare Customer Service Expert, you will be the first point of contact for patients needing home healthcare services. Typical phone calls range from interactions with customers who need basic information or to help those with more complex requests.
In this role, you will work in a remote high-volume call center environment and receive/respond to approx. 60-65 incoming calls per day from providers, referral sources and patients. You will collect clinical and demographic information and enter it into our CareCentrix portal.
