Overview

The Clinical Liaison, under the direction of the Director of Market Engagement, will drive performance and quality improvement initiatives across our skilled nursing facility (SNF) network. Serving as a vital link between acute and post-acute providers, this role collaborates with internal clinical operations teams and onsite SNF staff to analyze utilization trends, identify care gaps, and promote evidence-based best practices. The Clinical Liaison will educate stakeholders on industry standards, address barriers to success, and foster strong relationships to enhance patient outcomes and advance value-based care.

 

Using relationship-building, capability assessments, network analytics, and clinical expertise, you will support strategic initiatives such as reducing readmission rates, improving length of stay metrics, and promoting clinical integration across the care continuum.

Responsibilities

  • Lead and implement strategic projects to support care delivery improvements, including readmission rates, length of stay, optimizing length of stay, and other key performance metrics
  • Assess and document adherence to quality program requirements (e.g., protocols, measurements, and compliance tools)
  • Serve as a clinical resource and assist with resolving provider escalations through root cause analysis, problem solving and critical thinking
  • Facilitate clinical integration by providing tools, resources, and support such as EMR access, ADT feeds, and communication optimization
  • Deliver in-person education and training to network partners, focusing on data interpretation, performance trends, and industry standards
  • Develop and maintain a strong awareness and build relationships with the local community of healthcare providers (Acute, PAC Facility, and home-based services). Identify barriers to effective transitions of care, and support improved coordination between CareCentrix, SNFs, rounding physicians, and health systems
  • Provide regular onsite support to assigned skilled nursing facilities (SNFs) to strengthen collaboration, enhance care coordination, and foster effective relationship management

Qualifications

Education:

  • Bachelor’s degree in a healthcare-related field, or equivalent professional experience.
  • Active license required in one of the following: LPN, RN, or Social Worker (SW).

Experience:

  • Minimum of 3+ years in a healthcare setting, preferably within skilled nursing facilities (SNFs) or post-acute care environments.

Technical & Regulatory Knowledge:

  • Solid understanding of healthcare delivery systems, managed care regulations, contractual terms and stipulations, and governmental home health agency guidelines.
  • Proficient negotiation, communication, influencing, problem solving and decision-making skills.
  • Previous experience in utilization management or case management
  • Highly organized with excellent documentation and coordination capabilities

Clinical Competency:

  • Knowledge of CMS guidelines for SNF and LTCH criteria, or broader CMS guidelines for SNF, IRF, and LTCH criteria.
  • Advanced clinical knowledge with an emphasis on quality of care
  • Strong communication, customer service, and interpersonal skills
  • Demonstrated success in partnering with diverse, cross-functional teams and engaging stakeholders across all levels of the organization Effective problem-solving and decision-making abilities

Additional Requirements:

  • Willing and able to travel within the assigned geographic area. Please note: Relocation assistance is not offered for these positions
  • Willing and able to travel, by car or airplane, to locations within the assigned geographic region as required
