We are seeking a dedicated and experienced Registered Dietician or Registered Dietician Nutritionist to join our Readmission team at CareCentrix. In this role, you will play a crucial part in providing expert nutritional guidance and support to patients managing common chronic conditions. You will not only engage directly with patients but also lead the development and management of a comprehensive nutritional data repository to support our nurse coaches in delivering exceptional care.  

As a Social Services Coordinator you will work with the Nurse Coach team helping patients find resources and items they need upon discharge.   CareCentrix's extensive range of services provides support and coordination for patients and their families in every step of the healing process, including home health, durable medical equipment (DME), home infusion, sleep management, and care management services, which ultimately improve care transitions and reduce unnecessary hospital readmissions, and emergency room visits.

The PAC Nurse is a telephonic position responsible for managing the length of stay (LOS) for Long Term Acute Hospital (LTACH), Skilled Nursing Facility (SNF), and Institutional Rehab Facility (IRF) for their assigned post-acute care facilities through collaboration PAC Nurse will also collaborate with key facility personnel as well as with CareCentrix internal Medical Directors, Market Engagement Directors and Nurse Managers to develop and maintain a timely discharge plan.

As a Sr. Regulatory Compliance Analyst, you will monitor regulatory requirements impacting CareCentrix operations.  You will be responsible for analyzing and documenting requirements, communicating and educating business areas, developing training programs to ensure an understanding of requirements, and working with business areas to modify operations, policies, and procedures to ensure compliance and securing and maintaining CareCentrix’s operational licenses (PPO, UM, TPA).
