As a Nurse Coach you will utilize your clinical expertise to manage assigned Post-Acute Care patients’ transition from acute care setting to the home setting through telephonic outreach to provide education, coaching and care coordination to accomplish the goals of the Post-Acute Care Program. You will engage Post-Acute Care patients in the program and administer initial, update, and discharge assessments in an effort to prevent readmissions.  This position works closely with the Post-Acute Care patient, home health agency nurse, and physicians to facilitate education and adherence to establish health care goals and develop a personalized care plan. Position requires participation in performance and operational improvement activities. Works under moderate supervision.   Post-Acute Care is an end-to-end post-acute offering that manages episodes of care up to 90-days beginning prior to the patient’s discharge from the hospital. The program identifies the likely best site-of-care for the patient upon hospital discharge, manages length of stay (LOS) if a Skilled Nursing Facility (SNF) or Institutional Rehab Facility (IRF) is appropriate, and reduces hospital readmissions. Our program also coordinates the services required for a patient to transition to their home.

The Post-Acute Care Program assists with reducing hospital readmissions of clinically complex patients. The program couples patient education/coaching with Home Health interventions to assist a patient’s transition home after a hospital stay. The Analyst performs trending and analysis to support the provider service team with identifying trends and patterns that impact a provider’s cash flow. Works to complete research and resolve provider service issues related to claim payments to improve their outstanding A/R and to provide consistent payment patterns. This activity includes researching claim rejections and denials, portal connectivity issues, educate and communicate policy and process changes. Provides education to billing agencies and provider offices to ensure appropriate billing.
