As the Clinical Supervisor of the Post-Acute Care team you will be the primary point of contact for day to day clinical questions, concerns from internal and external clients and provides team leadership and guidance regarding patient centered interventions. Assists in development of Post-Acute Care training, job aids and tools and desk level procedures. Provides clinical oversight for the PAC Nurse Liasions.

As the Nurse Liaison you will be responsible for an assigned caseload at acute and post-acute care facilities. You will conduct in person, on site initial face to face outreach with patients to introduce the Post-Acute program, gather patient demographics and collaborate with hospital clinicians, patient and family to develop a discharge path of care. You will coordinate post-acute care services for the patient and collaborate with Post-Acute homecare agencies on post-acute care plan of care, authorizing services as medically necessary.
As the Medical Director of Post - Acute Care you will have clinical oversight of the Post-Acute Care (PAC) program for CareCentrix, performing the full range of PAC medical necessity reviews utilizing Industry standard criteria’s well as Client’s clinical criteria for Skilled Nursing Facility (SNF), Long Term Acute Hospital (LTACH)/Inpatient Rehabilitation Facility (IRF) as well as Home Health Service, DME and Home Infusion.
